A 14 yr male patient presented to OPD with a chief complaint of pain in abdomen,Vomiting,Fever since 4 days back
History of present Ilness
Patient was apparently asymptomatic since 5 days back and then developed fever associated with chills(which relived on medication)
• 2 Episodes of Black coloured stools in 1 day
•2 Episodes of vomiting which are non projectile and associated with food particles
•Associated with generalised weakness
•Sudden Onset of Giddiness
Past History
Not a known case of Diabetes Mellitus,Hypertension,Tuberculosis,Asthma,epilepsy
Family History
No similar complaints in the family

Bowel and bladder Movements:Normal
General Examination
No pallor
No icterus
No clubbing
No cyanosis
No lymphadenopathy
No pedal Edema
Vitals
Temperature:98.2F
Pulse rate:86
BP:100/70
Systemic Examination
CVS
S1 and S2 heard
No murmurs
CNS
Patient is conscious
Speech normal
Investigation
Mild ascitis and Mild Gall bladder edema

Viral pyrexia and Dengue NS1
Haemogram
Blood Urea
Serum Creatinine
ECG
Dengue Rapid Test
Final Diagnosis
Dengue with NS1 Antigen Positive
Treatment
IVF NS,RL@75ml/hr
Inj PAN 40mg IV/OD
Plenty of oral fluids
Soft oral diet
Inj optineuron 1 amp in 100 ml NS IV/OD
Strict BP monitoring 4 th hrly
